Suele comenzar despu\u00e9s de los 40\u201350 a\u00f1os y aumenta con la edad, la obesidad y lesiones previas de rodilla. Las mujeres presentan mayor riesgo y habitualmente es mas&nbsp; grave en comparaci\u00f3n con los hombres de forma comparative en el mismo rango de edad.<\/p>\n\n\n\n<p>-Desgaste del cart\u00edlago y formaci\u00f3n de osteofitos (picos \u00f3seos), con esclerosis del hueso subcondral e inflamaci\u00f3n sinovial intermitente, lo que genera dolor mec\u00e1nico y limitaci\u00f3n .<\/p>\n\n\n\n<p>&#8211; En radiograf\u00edas t\u00edpicamente se ve estrechamiento del espacio articular y osteofitos; los s\u00edntomas no siempre se correlacionan con la radiograf\u00eda.<\/p>\n\n\n\n<p>&#8211; Es la forma de artrosis m\u00e1s com\u00fan en el mundo y una causa mayor de discapacidad; en 2020 hubo ~595 millones de personas con artrosis, siendo la rodilla el sitio m\u00e1s frecuente, y el exceso de IMC aporta alrededor del 20% de la carga atribuible.<\/p>\n\n\n\n<p>&#8211; En poblaci\u00f3n \u226540 a\u00f1os, la prevalencia global estimada de artrosis de rodilla ronda el 22\u201323% y aumenta con la edad, m\u00e1s en mujeres.<\/p>\n\n\n\n<p>La artrosis de rodilla desgasta el cart\u00edlago por un desequilibrio entre \u201canabolismo\u201d (s\u00edntesis de matriz) y \u201ccatabolismo\u201d (degradaci\u00f3n), impulsado por citocinas, estr\u00e9s oxidativo y proteasas que rompen col\u00e1geno II y aggrecan.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><em>Proteasas que degradan la matriz<\/em><\/h3>\n\n\n\n<p>&#8211; MMP\u201113 (colagenasa) y MMP\u20111 rompen col\u00e1geno tipo II; su activaci\u00f3n genera fragmentos que amplifican la degradaci\u00f3n y muerte celular [1][2].<\/p>\n\n\n\n<p>&#8211; ADAMTS\u20114\/5 (\u201caggrecanasas\u201d) son clave en la p\u00e9rdida temprana de aggrecan; su actividad puede separarse temporalmente de MMPs, iniciando antes la degradaci\u00f3n de proteoglicanos [3][4].<\/p>\n\n\n\n<p>&#8211; La activaci\u00f3n pos\u2011traduccional de ADAMTS\u20114\/5 por convertasas como PACE4 aumenta su capacidad degradante in situ [5].<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><em>Citocinas y v\u00edas de se\u00f1alizaci\u00f3n<\/em><\/h3>\n\n\n\n<p>&#8211; IL\u20111\u03b2 y TNF\u2011\u03b1 aumentan MMPs y ADAMTS y reducen la s\u00edntesis de matriz; MAPK (p38, ERK1\/2) y STAT median estas respuestas [1][6][7].<\/p>\n\n\n\n<p>&#8211; IL\u20116 con su receptor soluble activa ERK\/STAT, induciendo MMP\u20111\/13, MMP\u20113 y ADAMTS\u20114\/5\/11 [7].<\/p>\n\n\n\n<p>&#8211; La sinovitis y mediadores inflamatorios del entorno articular perpet\u00faan la destrucci\u00f3n de la matriz [8].<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><em>Estr\u00e9s oxidativo y apoptosis del condrocito<\/em><\/h3>\n\n\n\n<p>&#8211; ROS (especies reactivas) y productos de peroxidaci\u00f3n lip\u00eddica como 4\u2011HNE modifican MMP\u201113 y favorecen degradaci\u00f3n; el bloqueo de v\u00edas pro\u2011oxidativas reduce apoptosis y MMP\u201113\/ADAMTS\u20115.<\/p>\n\n\n\n<p>&#8211; La hipertrofia\/apoptosis del condrocito y cambios en diferenciaci\u00f3n (COL10A1, caspasa\u20113) se asocian a degradaci\u00f3n temprana del cart\u00edlago.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><em>Mecanorregulaci\u00f3n y fragmentos de matriz (DAMPs)<\/em><\/h3>\n\n\n\n<p>&#8211; Carga mec\u00e1nica moderada suprime MMPs\/ADAMTS; sobrecarga o disuso promueven degradaci\u00f3n inducida por IL\u20111\/TNF\u2011\u03b1.<\/p>\n\n\n\n<p>&#8211; Fragmentos de fibronectina\/col\u00e1geno act\u00faan como DAMPs, activando integrinas\/TLRs y MAPK, lo que aumenta proteasas y NO.<\/p>\n\n\n\n<p>En conjunto, el cart\u00edlago se desgasta por activaci\u00f3n de MMP\u201113 y ADAMTS\u20114\/5 inducida por citocinas y estr\u00e9s oxidativo, junto con apoptosis\/hipertrofia de condrocitos y se\u00f1ales mec\u00e1nicas adversas; estos procesos reducen col\u00e1geno II y aggrecan y rompen la homeostasis de la matriz.<\/p>\n\n\n\n<p>En resumen, la artrosis de rodilla es un proceso degenerativo multifactorial de la articulaci\u00f3n que causa dolor y rigidez, muy prevalente globalmente, y cuyo manejo se centra en medidas no farmacol\u00f3gicas, f\u00e1rmacos sintom\u00e1ticos y, en casos avanzados, cirug\u00eda.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><em>Fuentes consultadas<\/em><\/h3>\n\n\n\n<p>[1] Charles J Malemud, Najmul Islam, &amp; Tariq M Haqqi (2003).<\/p>\n\n\n\n<p>Pathophysiological mechanisms in osteoarthritis lead to novel therapeutic strategies.
